CLIENT INFORMATION
Dear Client:
This information form was developed to answer questions that you may
have about The Animal Ophthalmology Center. The Animal Ophthalmology
Center is a Specialty Referral Ophthalmology Practice that is staffed
by a Veterinary Ophthalmologist licensed by the American Veterinary
Medical Association, Michigan Veterinary Medical Association, and Board
Certified by the American College of Veterinary Ophthalmologists.
Because The Animal Ophthalmology Center is a Referral Practice, we
are frequently referred complex and difficult cases for diagnosis and
treatment. Therefore, many of our patients require hospitalization in
order to complete the necessary tests to determine the causes of their
eye problems. You should be prepared to leave your animal if necessary.
Please arrive with your pet at The Animal Ophthalmology Center 15 minutes
prior to your scheduled appointment time to allow for check-in procedures.
You will be given an estimate of the costs involved for the diagnosis
(i.e., tests) and treatment of your animal's problem after your animal
has been examined. You should be prepared to make a deposit equal to
one half of the estimate cost of services for estimates greater than
$200 or the entire amount for estimates less than $200 if your animal
requires hospitalization. Final payment is due in full upon discharge
or conclusion of services. We are not able to offer payment plans. However,
we do accept checks drawn on a US bank, Mastercard, Visa or Discover
Card.
